Evaluate clients - 1 month

One month following your clinic, ask participants to complete a follow-up questionnaire. Use the feedback from this questionnaire to help adjust your course.

• In order to increase response, it is recommended that program participants be called back.
• Use the Graduation Questionnaire and Follow-Up Questionnaire Call Record to help organize and track calls to participants. Use one call record for each participant, to track when you called and the result: call was answered, message was left or no answer.
• If participants are smoke-free after one month, return their deposit plus their share of any additional funds from the program participant who are smoking at the time of the one-month follow.
